Review form responses and configure alerts

Review submissions and notify the right channel.

Before you start

The form must exist. Connect your Slack workspace before configuring Slack alerts.

Step by step

  1. Open Forms and select the form you want to review.
  2. Select Responses to view received submissions.
  3. Review submitted details and scores if scoring is configured.
  4. Under Alerts, select Add alert and define the conditions a response must meet.
  5. Choose a Slack channel from the list and save the configuration.
  6. Check that the published form uses the questions and conditions you want to collect.

A submission and an alert are separate outcomes

Responses shows received submissions. An alert also depends on its conditions and configured channel. A valid submission may not trigger an alert if it does not meet the selected score range. Check that relationship before changing the Slack connection.

Review the published form, scored options, and alert destination. Preview helps test the journey, but does not save a submission or create a Lead, so it is not a test of a live alert. To investigate a missing alert, use an existing submission and compare its score with the rule.

What to expect

You can review submissions and route configured alerts to the selected channel.

Troubleshooting

Responses are still saved when no alerts are configured.

If an alert is missing, check the score condition, channel, and Slack connection before changing the form.
