Build the questions you use to capture new contacts.
Before you start
You need Forms access. Decide what information you need and which answer leads to the next step.
Step by step
- Create a form in Forms and open its builder.
- Add questions, choose field types, and mark required questions. Map contact questions to the details they should save.
- Choose all questions at once, step-by-step, or logic mode. Configure branching when using logic.
- Review Appearance and endings: a completion screen or a redirect based on the score.
- Use the preview to complete the flow. Preview submissions do not create a Lead or save responses.
- Publish once you have reviewed the questions and outcome. Share the published URL or use an embed option.
Prepare your questions before publishing
Start with the information needed to identify the Lead, then add qualification questions. Short text, email, and phone fields serve different purposes: choose the appropriate type and check the contact-field mapping. The Required setting determines whether someone can continue without answering.
Preview lets you check the experience without creating real records. Test an incomplete response as well as each possible ending. In Logic mode, check every path; reviewing only the first question will not catch a broken jump. Saving and publishing are separate decisions: verify the public URL and the version visitors will see.
What to expect
You have a published form with a verified path and ending.
Troubleshooting
If publishing is unavailable, check that the form has questions. A correctly mapped email question helps match submissions to existing contacts.
